Ordering information: Orders may be placed via above email, telephone, or fax.
Please include the following information:
1. Purchase order number
2. Name of Institution
3. Shipping address, telephone, and end-user name
4. Billing address, telephone, and contact name
5. Catalog number, description, and quantity
6. Credit card type, account number, and expiration date (if applicable)

Shipping information: Products are shipped at ambient temperature via FedEx. Please store at recommended temperature as listed on technical data sheet upon arrival. There is a $30 shipping charge that is prepaid and added to invoice, or you may provide a FedEx account number at the time of order. Freight terms are F.O.B. destination.
